Student Athlete has read the above and agrees that he/she has been warned as to injury and accepts the responsibility of possible injury. I hereby consent to participation by my child, the Student Athlete named in this registration, in the sport described in this registration (“Sport”). In consideration of my child being allowed to participate in this sport, I hereby agree on behalf of myself and my child, to release Divine Child Catholic Schools and/or Church of the Divine Child, the Catholic Youth Organization (CYO), the Roman Catholic (Arch)diocese of Detroit, and any and all affiliated organizations, their employees, agents and representatives (Releasees), from any and all claims, including negligence, which may be asserted by me or my child, or on behalf of my child, arising from or relating to my child’s participation in the Sport. Athletes, parents and friends must be aware of our school's expectations with regard to sportsmanship. Unlike professional sports, as a spectator at an athletic event, you are a part of the activity, much like the athletes, coaches and officials. As a participant, (spectator or team member) we expect that you will maintain good sportsmanship or refrain from attending CYO athletic events. It is expected that as participants and spectators, we will support in a positive way our own team remembering that the athletes, coaches and officials are not perfect and will make mistakes. Negative, derogatory cheers or actions aimed at either team are not acceptable in educational athletics. It is expected that you will accept the decisions of the officials without vocalizing disagreement. Officials are an important part of this educational activity. We are sending the wrong message when we challenge or abuse the game official sent to the game to administer these educational activities. At all times it is expected that we will respect one another; adults and students alike. This especially includes our opponents and officials, without whose involvement, sport contests would not occur. Parental Code of ConductParents/Legal Guardians of student athletes must display conduct exemplary of the team, school, and Parish which they represent at all times. Conduct must demonstrate self-control, self-respect for others, and sportsmanship. This includes proper respect for and communication with all coaches, parents, officials, athletes and opponents, either in person or electronically. This applies at home and opponent's campuses. Parents/Legal Guardians must maintain a Christian attitude on and off the court/field. Should a meeting be required with a child's coach, an appropriate location and time should be determined by both parents and coach. In accordance with the principals of subsidiary, parents/legal guardians should address all disputes, concerns, questions, issues, and comments in the following order: 24 Hour Rule Meeting with Coach Meeting with Coach and Athletic Director Meeting with Coach, Athletic Director, and School Administrator Meeting with Coach, Athletic Director, School Administrator, and Pastor Meeting with Coach, Athletic Director, School Administrator, Pastor, and Head of CYO of Detroit (Athletics) Consequences for offenses may result in, but are not limited to: Written warning Removal from Athletic activities Student Athlete removed from Athletic TeamRequired* Parent/Legal Guardian acknowledges and agrees to abide by the above Parental Code of Conduct. Image and Audiovisual Release AuthorizationParent/Legal Guardian agrees that Church of the Divine Child and/or Divine Child Catholic Schools (collectively “Divine Child”) has the right to publish, reproduce and display photographic images, video images and/or audio recordings (collectively “images”) of their child, the Student Athlete. Parent/Legal Guardian understands that this material may be used in various publications, advertisements, displays, press releases, recruitment materials, broadcast public service advertising or other related endeavors. This material my also appear on Divine Child’s web pages or other social media outlets. Parent/Legal Guardian understands that images may be electronically displayed via the Internet or in the public setting. Parent/Legal Guardian understands that Divine Child may associate the photographic and/or video image or audio recording with the name of the Student Athlete and the name of the parish or school where Student Athlete is a member. Parent/Legal Guardian agrees that Divine Child shall have the unrestricted right to choose the media for display of their child’s, their Student Athlete’s, image.
